Paperwork Turn-in For all 2016 Football and Cheer Players
The season is right around the corner, but before your player can practice beginning August 1, we need to make sure all paperwork is 100% complete and turned-in.
Get your paperwork turned in now...players cannot practice August 1 until all paperwork is complete!
Paperwork requirements include:
- 2016 Player Contract
- 2016 dated physical
- 2015-2016 - end of year report card - no computer print outs from Infinite Campus will be accepted. Please email report cards to vicepresident@tvpw.org (or) you can submit in person.
- Copy of certified Birth Certificate
We will have league representatives available for any paperwork turn-in Thursday from 5-7pm at Birdsall Sports Park (during our pre-season conditioning and skills clinic).
As a reminder, we will have open conditioning and skills camp every Tuesday and Thursday (beginning July 7th) from 5-7pm. You may also get fitted for your football jersey during the conditioning and skills camp.
