We will only be offering physicals at 2 more events! The In-Person Registration on April 8 & our TVPW Clinic on May 19. Physicals conducted by TVPW are just $25.

• 2019 Player/Participant Contract

• 2019 Physical form

• Clear copy of athlete’s Birth Certificate

We’ll be on the main field by the blue storage containers - you can park along McCabe Dr and enter through the gates to access the field.
